How to obtain a U.S. Chamber of Commerce Approved Certificate of Origin from the EZCertOrigin:
Easy to follow step-by-step instructions:
1. Gather your invoice and bill of lading (if applicable). You will use these to complete the required forms.
2. Did your goods originate in the US?
- If yes, then use this form.
You MUST use the U.S. Chamber of Commerce's form and NOT another form.
- If no, then use this form.

- If your goods are of mixed origin, then follow this guideline:
If exporting a product with a majority of the materials being foreign, please use the Certificate of Non-US Origin form and break out the percentage of component parts (e.g., 20% U.S., 80% China). However, if the majority of the product is US origin, the exporter should contact the U.S. Department of Commerce at 1.800.USA.TRADE and ask whether the product qualifies as a U.S. origin product. If yes, then use the Certificate of U.S. Origin and do not break out the component parts.
3. Once you have the correct form, print it out, fill it in and sign it.

- Who is the owner or agent?
That is the individual who is authorized on behalf of the shipper to complete the form. It does not have to be the owner of the company. The person who is listed as the owner/agent must be the same person who signs the form.
- Where do I find the name of the carrier?
Look on the bill of lading for this information.
- What does consigned mean?
That means to whom the goods are going.
- What are the “Marks and Numbers”?
This is identifying information that helps separate your goods from other goods. For example, this could be an item number and/or the invoice number. This information is usually found on the invoice.
- What do I put for a description of item?
This needs to match the description on the invoice for the item. It is preferable to copy the entire description of the item directly from the invoice.
4. After you have completed the form, find the appropriate affidavit and print it out.
- If your goods originated in the United States, use this form.
- If your goods did not originate in the United States, use this form.
- Check to ensure that you listed the country of origin if it is not the United States. You may list multiple countries, if applicable.
5. Once you have completed the affidavit, you must have it notarized.

- I am a notary. Can I sign and notarize the form myself?
No. The notary must be different from the owner/agent.
- Where can I find a notary?
If your company does not have a notary, you can often find one at your bank who will notarize the items for a nominal fee.
6. An invoice AND a Bill of Lading (if applicable) for the goods you list on the certificate.
7. Make appropriate payments for the certificates. Your certificate will not be processed until we have received a certified check or your credit card is processed ($5 credit card processing fee). The check must be made out to ICS Consulting, LLC. For credit card orders, please click here.
The fee for each certificate is:
- $40 each for a US Chamber member certificate fee
(Member number MUST be submitted to receive this price.)
- $150 each for a Non-member certificate fee
- $5 each for copies of Certificate of Origin
- $5 each for an Invoice stamped with U.S. Chamber seal
- $5 each for copies of stamped invoice needed (Only those stamped with seal will be copied.)
- $5 each for a Packing list stamped with U.S. Chamber seal
- $5 each for copies of stamped packing list needed (Only those stamped with seal will be copied.)
- $28 for returning documents via FedEx.

8. Fill out the customer information form. This form must be completed and included in certificate request for processing to be completed.
9. Optional: If you are a first-time exporter, we strongly recommend that you scan and email us a draft of the documents for same day review. This pre-review will expedite the process and ensure that you do not have to mail multiple copies of documents, or spend excess money on shipping incorrect documents. 10. Once you have completed these items, review the checklist. Once you are sure you have everything, mail the documents to the following address. Mail to:

11. Once we receive the documents, you will receive an email from us indicating your order is processing. We will also contact you if any changes are needed on your forms.
12. Once the forms are correct, the completed certificates of origin will be shipped within 24-48 hours.
